I authorize the Pet Sitter, Anthony Mucci, to obtain emergency medical services in the event I cannot be reached. I will be fully responsible for all fees and charges and will pay for all charges incurred on my behalf upon my return.

Client shall pay the pet sitter, Anthony Mucci, in full upon completion of services rendered.
Compensation Policy:
In addition the Client will reimburse the pet sitter, Anthony Mucci, for any and all necessary costs incurred by the pet sitter for emergency pet care.
Client will be responsible for any and all bank fees due to returned checks.

Private Cage Free Pet Boarding Agreement
I affirm that my pet is clean, recently bathed and on flea and tick control product at this time. Note: Trifexis & Comfortis only control fleas.

I will provide food for my dog(s) and any treats they are allowed upon arrival at pet sitter's home.

I agree to pick up my pet(s) and pay the Pet Sitter in a timely manner. If I fail to pick up my pet(s) by the agreed deadline and do not contact the Pet Sitter to obtain an extension, in such an event as the Pet Sitter is unable to contact me by any of the methods of personal contact I have provided within ten (10) days of the pick up deadline date, I understand that the Pet Sitter reserves the right to consider my pets abandoned and may surrender them to the local Human Society or Animal Control Office at their own discretion.

I agree that Pet Sitter can’t be held liable for the injury or death of my pet due to illness or natural cause.

I understand that Pet Sitter will, and is hereby given permission, to make every diligent effort to obtain emergency veterinary care for my pet if it should become ill or injured, but will not hold Pet Sitter liable for such illness or injury that occurs to my pet while under their care.

I will not attempt to hold the Pet Sitter responsible nor liable for any injury or illness to my pet following their care by the Pet Sitter.

I understand that I, and not the Pet Sitter, will be held responsible if my pet bites any human or animal while under the Pet Sitter’s care.

I acknowledge awareness that any animal biting a human must be reported to the authorities and the animal will be quarantined to ensure no threat of rabies. I will not hold the Pet Sitter responsible for liability or loss resulting from any action necessitated following my pet biting a human or animal while under the Pet Sitter’s care. The Customer is liable for any and all damages caused by their pet while boarding at the pet sitters location.

Client will be billed for and expected to pay for any and all repairs including but not limited to rugs and furniture if destroyed by the clients’ pet, if no warning given to the pet sitter about the pets destructing behavior.

The information contained in this service agreement is for company use only. This information will not be used for any purpose other than to conduct Daily Dog Walkers & Pet Sitters business.

Both parties agree that the engaged Pet sitter is to perform all services at the pet sitters’ premises, unless agreed to otherwise. The Pet Sitter is willing to contract with the customer, and the Customer is willing to contract with the Pet Sitter, on the terms and conditions hereinafter set forth. For the reasons set forth above, and in consideration of the mutual covenants and promises of the parties hereto, Pet Sitter and Customer covenant and agree as follows:

WAIVER OR MODIFICATION INEFFECTIVE UNLESS IN WRITING

No waiver or modification of this agreement or of any covenant, condition, or limitation herein contained shall be valid unless in writing and duly executed by the party to be charged therewith. Furthermore, no evidence of any waiver or modification shall be offered or received in evidence in any proceeding, arbitration, or litigation between the parties arising out of or affecting this agreement, or the rights or obligations of any party hereunder, unless such waiver or modification is in writing, duly executed as aforesaid. The provisions of this paragraph may not be waived except as herein set forth.

CONTRACT GOVERNED BY LAW

This agreement and performance hereunder and all suits and special proceedings hereunder shall be construed in accordance with the laws of the State of Florida Executed on the date first above written.
